- Offline Assistance Forms: Our Funding Request Form, Resource Order Form, and Scholarship Application are downloadable PDFs. These are submitted directly through hospital social workers, advocates, text, fax, or mail—not through our website.
- Device & Usage Data: IP address, approximate location, browser/OS type, and interaction data (pages viewed, links clicked).
Donations
Donations are processed externally through Give Lively at givelively.org under its own privacy policy. We do not collect, transmit, or store payment information on cancerkidssjc.org.
How We Use Information
We use this information to respond to inquiries and support website analytics and performance.

- Service Providers: Website hosting and form processing vendors operating on our behalf.
